At Montgomery County Schools, the health, safety, and well-being of every student are essential to their success in the classroom. Our Health Services team is committed to providing a safe and supportive environment where students can learn, grow, and thrive.
Whether you're looking for information about immunization requirements, medications at school, health forms, chronic medical conditions, or guidance on when your child should stay home, you'll find the resources you need here. We encourage families to partner with our school nurses and health staff to ensure students receive the care and support they need throughout the school year.

The health forms below are available for you to download and print as needed. Individual Health Plans (IHPs) must be completed each school year. After completing the appropriate form(s), please return them to your child's school nurse or school office.
If you do not have access to a printer, printed copies are available from your child's school nurse. Please contact your child's school, and we'll be happy to assist you.

Montgomery County Schools partners with Sterling Health Care to provide school-based healthcare services for students during the school day. To allow your child to receive care from a Sterling Health provider at school, a new School-Based Health Services Consent Form must be completed by a parent or guardian each school year.

Completing this form does not require your child to receive services, but it ensures they have access to care when needed throughout the school year.
